Age Requirement

All clients under the age of 18 must have parental consent. I welcome parents to join during consultation also, this helps in knowing full health history throughout childhood.

Booking and Changing appointments

You can book all appointments by phoning 1-778-985-7499 or by reaching me via email hello@allysonobrien.ca .
A credit card, gift card or prepayment is required at the time of booking to hold all appointments. Please advise of changes or cancellations to the reservation 24 hours prior to your reservation via phone or email to hello@allysonobrien.ca . Respectfully, late cancellations or changes incur the full appointment fee.

Arrival

Please arrive 5 minutes prior to your scheduled appointment time. Treatment deposits are non-refundable if patients fail to appear. A “no- show” fee will apply if you fail to appear to your confirmed appointment time. Credit cards are required at time of booking for all treatments.

Clinic Etiquette

Whilst in the centre, I ask that you kindly respect the staff, facilities, policies and the enjoyment of other guests. All cell phones are to be turned off or to silent while within the wellness center walls. Forms of payment will be accepted in forms of Cash, Debit, Visa, Master Card, Amex and Interac E-Transfers. I am committed to ensuring wellness and skin care satisfaction.

Health Conditions

Prior to any treatment, please use our consultation forms to disclose to Allyson O’Brien of Allyson O’Brien Nutrition & Advanced Aesthetics your medical history, including any medical conditions, per-existing injuries, infectious diseases, skin conditions, allergies, and pregnancy. If your health conditions have changed since first completing the forms, please advise as soon as possible and prior to your next treatment to allow us to make the necessary adjustments.

For any additional dietary requirements, allergies, conditions (including pregnancy) or disabilities not advised at the time of making the reservation, please contact me so I can assist in making the appropriate choices or modifications. Whilst all care is taken, I can not guarantee any of her treatments are completely free from traces of allergens, or appropriate for any medical condition. Please seek prior advice from your doctor or healthcare professional.

Patients being treated for chronic medical conditions or cancer should provide a written clearance from their doctor. Due to increased sensitivity and various hormonal changes during pregnancy, there are some treatment contraindications and special precautions for pregnant and breastfeeding mothers. If you have any concerns regarding sensitivity, please consult your doctor prior to booking.
